不同军事训练项目引发BPPV半规管类型、治疗及复发的临床对比
Clinical comparison of semicircular canal types,treatment and recurrence of BPPV triggered by different training programs

Abstract
Objective To report semicircular canals involved,initial treatment efficacy and recurrence of benign paroxysmal positional vertigo(BPPV)associated with different military training programs.Methods Subjects included 213 patients(176 males and 37 females,aged 19 to 42 years[mean=31.40±6.40 years])with BPPV diagnosed from June 2022 to June 2024 at the 989th Hospital of the Joint Logistics Support Force,which occurred during training.The SRM-Ⅳ vestibular function diagnosis and treatment system was used for reduction therapy,with follow-ups at 2 weeks,2 months,4 months,and 6 months to assess efficacy of the initial and relapses.Results Among the 213 cases,BPPV occurred during conventional training programs in 21,involving the posterior semicircular canal in 13,horizontal semicircular canal in 6,and more than one semicircular canal in 2.Nineteen patients(90.5%)improved after initial treatment,with two relapsed(9.5%).BPPV occurred during conventional training combined with blast exposure in 133 cases,involving the posterior semicircular canal in 81 cases,horizontal semicircular canal in 34 cases,anterior semicircular canal in 1 case,and more than one semicircular canal in 17 cases.Initial treatments were effective in 114 cases(85.7%),with 31 relapses(23.3%).In 59 cases,BPPV occurred during conventional training program plus hand-to-hand combat training,involving the posterior semicircular canal in 26 cases,horizontal semicircular canal in 10 cases,anterior semicircular canal in 2 cases,and more than one semicircular canal in 21 cases.Favorable response to initial therapeutic maneuvers was observed in 50 cases(84.7%),with relapses in 21 cases(35.6%).Compared to other populations,military personnel exhibit better responses to the initial treatments for military training associated BPPV independent of training programs,with less recurrence which can vary with different training programs.Conclusions The number of effective cases for the first treatment of BPPV patients caused by military training is not related to the different types of training programs.The type of semicircular canal involved and recurrence are related to different types of training programs.关键词
