"筋骨平衡"下筋针疗法联合手法治疗颞下颌关节紊乱病的疗效分析
Efficacy Analysis of Tendon Needling Therapy Combined with Manipulative Techniques for TMD under Concept of Tendon-Bone Balance

Abstract
Objective:To observe the clinical efficacy of tendon needle therapy combined with manipulative techniques in treatment of temporomandibular joint disorder(TMD).Methods:A total of 126 patients with TMD were randomly divided into the combined therapy group and the manipulative techniques group.The manipulative techniques group was treated with orthopedic manipulation primarily based on the rotational cervical technique;on which basis,the combined therapy group was also treated with tendon needling.Both groups underwent interventions for 4 weeks.Pain intensity was assessed using the Visual Analog Scale(VAS)at baseline and at 1,2,and 4 weeks after intervention.Temporomandibular joint function was evaluated using joint range of motion and the Fricton index at baseline and after 4 weeks.Daily living activity and quality of life were assessed using the Mandibular Function Impairment Questionnaire(MFIQ)and the Chinese version of the Oral Health Impact Profile-14(OHIP-14).Electromyographic signals of the affected and unaffected sides were recorded before and after the intervention,and overall clinical efficacy was evaluated.Results:In terms of VAS scores,there was a significant interaction between treatment method and time(P＜0.05);except for the 1-week measurement in the manipulative technique group,VAS scores at all time points after intervention were significantly lower than baseline in both groups(P＜0.05);the combined therapy group showed lower VAS scores than the manipulative technique group at 1,2,and 4 weeks post-treatment(P＜0.05).In terms of Fricton index,the combined therapy group had lower JP,MP,MM,DI,PI,and CMI than the manipulative technique group(P＜0.05),except for the JN parameter;MMO and RMS on the affected side were higher in the combined therapy group compared to the manipulative technique group(P＜0.05);scores of MFIQ and OHIP-14 were lower in the combined therapy group(P＜0.05).The clinical efficacy between the two groups was analyzed wsing the rank-sum test.The combined therapy group showed significantly better dinical efficacy than the manipulative techniques group(P＜0.05).Although the total effective ratc in the combrired therapy group was 98.41％(62/63)which was higher than 96.72％(59/61)in the maipllative group,the difference was not statistically significant(P＞0.05).Conclusion:Tendon needling therapy combined with orthopedic manipulation can effectively alleviate pain,improve functional impairment,and enhance daily living ability and quality of life in patients with TMD.关键词
