Abstract
Spinal cord injury(SCI)is a highly disabling neurological disorder whose pathological mechanisms involve primary mechanical damage as well as complex secondary processes,including inflammatory cascades,oxidative stress,and glial scar formation.Recent studies have shown that acupuncture can modulate the spinal cord microenvironment through multiple targets and pathways,demonstrating considerable potential for neural repair.Among these mechanisms,the stromal cell-derived factor-1α(SDF-1α)/C-X-C chemokine receptor 4(CXCR4)signalling pathway has attracted increasing attention as a key regulatory axis.This study systematically summarizes the mechanisms by which acupuncture promotes spinal cord repair via regulation of the SDF-1α/CXCR4 pathway,including enhancement of stem cell migration and homing,improvement of the inflammatory microenvironment,and facilitation of neuro-regeneration and synaptic remodelling.Future studies should focus on the precise optimization of acupuncture parameters,the potential risks associated with excessive activation of SDF-1α,and mechanistic validation through multi-technique approaches.Further in-depth investigation into the spatiotemporal specificity of acupuncture-mediated regulation of the SDF-1α/CXCR4 pathway,as well as its synergistic effects with traditional Chinese medicinal,electrical stimulation,and other therapies,may provide novel strategies for the precision treatment of SCI.关键词
