Exploring the Risk Factors for Recurrence of Bacterial Vaginitis in Elderly Women Based on Vaginal Microecology

Abstract
Objective:To explore the risk factors for the recurrence of senile vaginitis based on vaginal microecology,and to provide a basis for reducing the recurrence rate of senile vaginitis in clinical practice.Method:A retrospective selection was made of 80 elderly patients with vaginitis admitted to the First Affiliated Hospital of Qiqihar Medical University from January 2025 to June 2025 as the research subjects.Vaginal secretion samples were collected in the first month after the patients were cured.According to the recurrence situation,the patients were divided into the recurrence group(38 cases)and the non-recurrence group(42 cases).The general data of the two groups were compared,and multivariate logistic regression analysis was used to analyze the influencing factors of recurrence in elderly patients with vaginitis.Result:There were statistically significant differences in the density of microecological flora,flora diversity,Lactobacillus classification,pH value,T lymphocytes,B lymphocytes,natural killer(NK)cells,Nugent score,and Aerobic Vaginitis(AV)score between the two groups(P<0.05).There were no statistically significant differences in age,body mass index,type and medical history between the two groups(P>0.05).The recurrence of elderly patients with vaginitis was taken as the dependent variable,and the indicators with statistical significance in the univariate analysis were taken as independent variables for multivariate logistic regression analysis.The results showed that Microecological flora density not at gradeⅡ～Ⅲ(OR=1.084),flora diversity not at grade Ⅱ～Ⅲ(OR=1.085),Lactobacillus grade not at grade Ⅰ～Ⅱa(OR=1.093),pH>4.5(OR=1.092),low CD4+(OR=1.082),low CD8+(OR=1.081),Nugent score≥4 points(OR=1.090)and AV score≥4 points(OR=1.088)are the influencing factors for recurrence in elderly patients with vaginitis(P<0.05).Conclusion:The factors influencing the recurrence of elderly patients with vaginitis include non-grade Ⅱ～Ⅲ microecological flora density,non-grade Ⅱ～Ⅲ flora diversity,non-grade Ⅰ～Ⅱa Lactobacillus classification,pH>4.5,low CD4+,low CD8+,Nugent score≥4 points,and AV score≥4 points.Clinically,the above indicators can be used to predict the recurrence of elderly patients with vaginitis and provide prognosis for the patients.关键词
