深圳宝安沙井、新桥两街道高血压患者不同年龄阶段的中医证型分布特点相关研究
The Study on the Distribution Characteristics of Traditional Chinese Medicine Syndromes in Hypertensive Patients of Different Age Groups in Shenzhen Bao'an Sha Jing and Xing Qiao Districts

Abstract
Objective:To investigate the distribution characteristics of Traditional Chinese Medicine(TCM)syndromes and their influencing factors in hypertensive patients of different age groups in Shenzhen Bao'an Shajing and Xinqiao Districts.Method:A retrospective study was conducted on 450 hypertensive patients who visited community health service centers in Shajing and Xinqiao subdistricts of Shenzhen Integrated Traditional Chinese and Western Medicine Hospital between October 2022 and March 2023.They were divided into a youth group(18～39 years),a middle-aged group(40～59 years),and an elderly group(60～80 years)based on age.Basic information,blood glucose,blood lipids,and other indicators were collected for all patients,and TCM syndrome differentiation was performed(Liver Fire Hyperactivity syndrome,Yin Deficiency with Yang Hyperactivity syndrome,Phlegm-Dampness Abundance syndrome,Yin-Yang Deficiency syndrome).Multivariate logistic regression model was used to analyze the influencing factors of TCM syndromes in different age groups,and the predictive performance of the models was evaluated using Receiver Operating Characteristic(ROC)curve.Result:There were statistically significant differences among the three groups in terms of baseline data,blood glucose and lipid levels,and distribution of traditional Chinese medicine syndromes(P<0.05).TCM syndrome distribution showed the youth group was predominantly Liver Fire Hyperactivity type,the middle-aged group was mainly Phlegm-Dampness Abundance type and Liver Fire Hyperactivity type,while the elderly group was primarily Yin-Yang Deficiency type and Phlegm-Dampness Abundance type.Multivariate logistic regression analysis showed smoking,alcohol consumption,and high TG levels were independent risk factors for Liver Fire Hyperactivity syndrome in the youth group.High BMI and a history of high-salt diet were independent risk factors for Phlegm-Dampness Abundance syndrome in the middle-aged group.Increasing age,comorbid diabete,and low HDL-C levels were independent risk factors for Yin-Yang Deficiency syndrome in the elderly group.ROC curve analysis showed that the AUCs for the three prediction models were 0.834,0.796,and 0.721,respectively,all indicating good discriminatory performance.Conclusion:The distribution of TCM syndromes in hypertensive patients in Shenzhen Bao'an Shajing and Xinqiao Districts,shows significant age-stage differences with complex and diverse influencing factors.Establishing age-specific syndrome prediction models is helpful for achieving precise TCM syndrome differentiation and treatment of hypertension.关键词
