Clinical and prognostic significance of Syndecan-2 expression in diffuse large B-cell lymphoma

Abstract
Objective To investigate the expression of Syndecan-2(SDC2)in diffuse large B-cell lymphoma(DLBCL)and to evaluate its clinicopathological significance,prognostic value,and association with the tumor microenvi-ronment(TME).Methods Transcriptomic data from the Gene Expression Omnibus(GEO)dataset(GSE53786)were analyzed to assess the relationship between SDC2 expression,patient prognosis,and tumor microenvironment scores.In addition,tissue samples from 137 patients with DLBCL were collected,and SDC2 protein expression was evaluated by im-munohistochemistry.Associations between SDC2 expression and clinicopathological characteristics,overall survival(OS),and immune cell infiltration were further analyzed.Results Bioinformatics analysis revealed that high SDC2 expression was an independent protective factor for overall survival in patients with DLBCL(HR=0.40,95％CI:0.23-0.72,P=0.002).Moreover,SDC2 expression showed a significant positive correlation with stromal scores(R=0.621,P＜0.001).Immunohistochemical validation confirmed that patients with high SDC2 expression had significantly better overall survival(P=0.021).Multivariate analysis further demonstrated that SDC2 remained an independent protective prognostic factor(HR=0.469,95％CI:0.226-0.974,P=0.042).In addition,a trend toward reduced infiltration of CD68+macrophages was observed in the SDC2 high-expression group.Conclusion SDC2 serves as an independent favorable prognostic biomarker in DLBCL,with higher expression associated with prolonged overall survival.Its prognostic signifi-cance may be related to modulation of the tumor microenvironment,particularly through effects on stromal components.关键词
