Evaluation value of teacher version of Swanson,Nolan and Pelham rating scale,version Ⅳ in school-aged children with attention deficit and hyperactivity disorder

Abstract
Objective To explore the evaluation value of the teacher version of Swanson,Nolan and Pelham rating scale,version Ⅳ(SNAP-Ⅳ)in school-aged children with attention deficit and hyperactivity disorder(ADHD).Methods Children who visited Liaocheng People's Hospital from October 2023 to August 2024 with chief complaints of inattention,hyperactivity,or learning difficulties were enrolled.Diagnostic data based on the Diagnostic and statistical manual of mental disorders,fifth edition(DSM-5)and assessment data from the teacher version of the SNAP-IV were collected.The Kappa test was used to analyze the consistency between the teacher version of SNAP-IV and DSM-5 in the diagnosis of ADHD and its subtypes.Receiver operating characteristic(ROC)curve analysis was performed to determine the sensitivity and specificity of the teacher version of SNAP-IV in diagnosing ADHD subtypes.Results Kappa test showed that the consistency between different positive criteria of the teacher version of SNAP-IV and DSM-5 diagnosis of ADHD was low(κ value≤0.537),and the consistency in diagnosing ADHD subtypes was even lower(κ value<0.400).ROC curve analysis revealed that when the mean score cutoff of the attention deficit subscale of the teacher version of SNAP-IV was 1.083,the sensitivity and specificity for diagnosing attention deficit were 0.856 and 0.800,respectively.When the mean score cutoff of the hyperactivity-impulsivity subscale of the teacher version of SNAP-Ⅳ was 0.860,the sensitivity and specificity for diagnosing hyperactivity-impulsivity were 0.814 and 0.673,respectively.Conclusion The teacher version of SNAP-Ⅳ has clinical evaluation value for school-aged children with ADHD,but its consistency with DSM-5 in classifying ADHD subtypes is low.关键词
