独活寄生汤加减治疗对老年退行性腰椎管狭窄症腰椎减压融合内固定术后残余疼痛的疗效
Effect of modified Duhuo Jisheng decoction on residual pain after lumbar decompression fusion and internal fixation for senile degenerative lumbar spinal stenosis

Abstract
Objective To investigate the effect of modified Duhuo Jisheng decoction on residual pain after lumbar decompression and fusion for elderly degenerative lumbar spinal stenosis(DLSS).Methods A total of 152 patients with residual pain after DLSS diagnosed and treated in this hospital from March 2021 to February 2025 were selected as the research objects.A total of 152 patients were divided into control group and experi-mental group by random number table method,with 76 cases in each group.Both groups received routine re-habilitation training.The control group was treated with conventional western medicine on the basis of routine rehabilitation training,and the experimental group was treated with Duhuo Jisheng decoction on the basis of the control group.The efficacy,laboratory indexes[serum substance P(SP),prostaglandin E2(PGE2),5-hydroxytryptamine(HT),plasma viscosity,whole blood high shear viscosity,whole blood low shear viscosity]levels were compared between the two groups after 4 weeks of treatment.The visual analogue scale(VAS)scores of the two groups were compared before treatment,4 weeks and 3 months into treatment.The occur-rence of adverse reactions in the two groups was recorded.Results The total effective rate of the experimental group was 93.42%,which was higher than 78.95%of the control group,and the difference was statistically significant(P<0.05).After treatment,the serum levels of SP,PGE2 and 5-HT,plasma viscosity,whole blood high-shear viscosity,whole blood low-shear viscosity and the total scores of main symptoms,secondary symptoms and TCM syndromes in the two groups were lower than those before treatment,and those of the experimental group were lower than those of the control group,and the differences were statistically signifi-cant(P<0.05).After 4 weeks and 3 months of treatment,the VAS scores of rest and activity in the two groups were lower than those before treatment(P<0.05),and the VAS scores of the experimental group af-ter 4 weeks and 3 months of treatment were lower than those of the control group,the differences were statis-tically significant(P<0.05).There was no significant difference in the total incidence of adverse reactions be-tween the two groups(P>0.05).Conclusion Duhuo Jisheng decoction is effective in the treatment of DLSS in the elderly.It can down-regulate the expression of pain mediators and reduce plasma viscosity,so as to re-lieve and prolong the analgesic effect,without increasing adverse reactions,and has good safety.关键词
