基于古籍探析从中焦论治妇科癥瘕
Treatment of Gynecological Abdominal Mass from Middle Jiao Based on Ancient Texts

Abstract
Gynecological abdominal mass is a common clinical disorder,primarily located in the lower Jiao.The pathological products responsible for its development are food retention,phlegm-turbidity,and blood stasis,which mainly arise from dysfunction of the middle Jiao.The middle Jiao governs the transformation of food and fluids and the generation of qi and blood,processes that depend on the smooth regulation of qi movement.The ascending function of the spleen and the descending function of the stomach,together with the ascending of the liver and descending of the gallbladder,play major roles in the regulation of qi movement in the middle Jiao.Dysfunction of the middle Jiao's transformation and imbalance of qi movement are key factors in the formation of food retention,blood stasis,and phlegm-turbidity.Specifically,dysfunction of the spleen and stomach in transformation tends to generate food retention and phlegm-turbidity,whereas impaired dispersion and regulation of the liver and gallbladder tends to produce blood stasis,phlegm-turbidity,and food retention.In the treatment of gynecological abdominal mass from the perspective of the middle Jiao,in cases of deficiency of form and qi,priority should be given to strengthening the spleen and benefiting the stomach;in cases of excess of form and qi,priority should be given to eliminating accumulated pathogenic factors.Soothing the liver and relieving depression,as well as regulating emotions,should be integrated throughout the entire course of treatment for gynecological abdominal mass.Based on ancient texts,this paper discusses the pathogenesis and treatment of gynecological abdominal mass from the perspective of the middle Jiao,with the aim of providing a theoretical reference for clinical practice.关键词
