Evaluation of the impact of episiotomy on short-term pelvic floor function in primiparous women with full-term delivery using 3D ultrasonography combined with clinical pelvic floor testing

Abstract
Objective:To comprehensively evaluate the impact of episiotomy on short-term pelvic floor function in primi-parous women after full-term delivery using three-dimensional pelvic floor ultrasound combined with clinical pelvic floor as-sessment.Methods:A retrospective study was conducted on 60 primiparous women who delivered vaginally at Nanning First People's Hospital between June 2024 and April 2025.Participants were divided into two groups:the study group(with epi-siotomy,n=30)and the control group(without episiotomy,n=30).All subjects underwent 3D pelvic floor ultrasound,pelvic mus-cle strength and pressure assessment,and POP-Q staging at 42～60 days postpartum.Differences in various indicators between the two groups were compared.Results:No significant differences were observed between the two groups in general character-istics,pelvic muscle strength,dynamic pressure,most POP-Q parameters,or ultrasound measurements(P>0.05).However,the total vaginal length(TVL)in the study group was significantly longer than that in the control group(P=0.026).Additionally,the position of the uterine cervix during maximum Valsalva was significantly lower in the study group(P=0.025 5),and the inci-dence of uterine prolapse in the study group was significantly higher than that in the control group(P=0.032).Conclusion:Episiotomy can not significantly improve short-term pelvic floor function and may be associated with an increased risk of u-terine prolapse,indicating its limited protective effect on the pelvic floor.Therefore,episiotomy should be strictly controlled and not be used as a routine procedure in vaginal delivery.关键词
