脉冲电场消融在心房颤动治疗中的应用:从理论探索到临床实践
Application of pulsed field ablation in the treatment of atrial fibrillation:from theoretical exploration to clinical practice
摘要
Abstract
Atrial fibrillation(AF)is the most common tachyarrhythmia,with its incidence increasing year by year with population aging,imposing a heavy disease burden on patients and society.Catheter ablation is an important method for rhythm control of AF.Although traditional radiofrequency ablation is widely used,it has inherent defects such as steam pop and adjacent tissue injury.As a novel non-temperature-dependent ablation technology,pulsed field ablation(PFA)exhibits unique advantages in reducing complications due to its characteristic of selective electroporation of myocardial tissue.This article systematically reviews the technical principles,early basic experiments,clinical exploration,and large-scale real-world research data of PFA,focusing on analyzing its efficacy and safety in the treatment of paroxysmal and persistent AF.It also discusses the current key issues such as coronary artery injury,device heterogeneity,and additional ablation strategies,and finally looks forward to the development direction of PFA in the fields of optimization of three-dimensional navigation,innovation of surgical mode,and postoperative management,aiming to provide references for clinical practice and scientific research.关键词
