A Study on the Trajectory of Anxiety and Depression and Influencing Factors in Patients with Lymphoma During Chemotherapy
摘要
Abstract
Objective To analyze the trajectory and influencing factors of anxiety and depression in patients with lymphoma during chemotherapy. Methods A total of 58 patients with lymphoma(n=58)who received chemotherapy at Tangdu Hospital of Air Force Medical University from March 2023 to February 2024 were selected as study subjects.A general information questionnaire was used to collect demographic and clinical data before chemotherapy initiation(T0).The Hospital Anxiety and Depression Scale(HADS)was used to assess anxiety and depression levels at T0,at the end of the first chemotherapy(T1),at the end of the second chemotherapy(T2),at the end of the third chemotherapy(T3),at the end of the fourth chemotherapy(T4),and at the end of the fifth chemotherapy(T5).Generalized estimating equation models were applied to analyze the influencing factors of anxiety and depression in patients with lymphoma during chemotherapy. Results The detection rates of anxiety at T0 to T5 were 70.69%,94.83%,96.55%,79.31%,44.83%,and 15.52%,respectively,and the detection rates of depression were 65.52%,89.66%,94.83%,75.86%,39.66%,and 12.07%,respectively.The scores of HADS-Anxiety subscale(HADS-A)at T0 to T5 were 9.22±1.51,12.07±2.58,14.83±2.23,10.40±1.72,6.76±1.37,and 2.67±0.80,respectively,and the scores of HADS-Depression subscale(HADS-D)were 8.93±1.66,11.40±1.72,14.14±2.67,9.69±1.70,6.47±1.84,and 2.59±0.79,respectively.Generalized estimating equation model analysis showed that educational level(OR=0.460,95%CI:0.295-0.625),family per capita monthly income(OR=0.489,95%CI:0.352-0.679),surgical history(OR=2.408,95%CI:1.647-3.523),and disease stage(OR=2.547,95%CI:1.711-3.792)were influencing factors for anxiety in patients with lymphoma during chemotherapy(P<0.05);gender(OR=1.914,95%CI:1.401-2.613),educational level(OR=0.469,95%CI:0.330-0.666),marital status(OR=2.298,95%CI:1.580-3.341),and family per capita monthly income(OR=0.389,95%CI:0.260-0.582)were influencing factors for depression in patients with lymphoma during chemotherapy(P<0.05).Conclusion Anxiety and depression symptoms are common in patients with lymphoma during chemotherapy. The scores of anxiety and depression show a trajectory of increasing first and then decreasing, peaking in the middle of chemotherapy, and are associated with educational level, family per capita monthly income, surgical history, disease stage and other factors.关键词
