Abstract
Objective To investigate the predictive value of platelet parameters combined with clinical indicators for bronchopulmonary dysplasia(BPD)in premature infants.Methods A retrospective analysis was conducted on 272 premature infants admitted to Hefei Maternal and Child Health Hospital from January 2020 to January 2025.According to the BPD diagnostic criteria,they were divided into BPD group(n=133)and non-BPD group(n=139).General clinical data and platelet parameters,including platelet count(PLT),platelet distribution width,plateletcrit,and mean platelet volume,measured on postnatal day 1 and at weeks 1,2,and 3,were compared between two groups.Binary Logistic regression and receiver operating characteristic curve were used to evaluate the predictive value of clinical characteristics combined with platelet-related parameters for BPD.Results Binary Logistic regression analysis identified gestational age(OR=0.93),PLT at week 3(OR=0.99),pulmonary surfactant(PS)administration(OR=2.41),and duration of caffeine therapy(OR=1.03)as independent predictors of BPD(P<0.05).The combination of these four factors predicted BPD with an area under the curve of 0.802,a sensitivity of 79.7%,and a specificity of 68.3%.Conclusion Reduced PLT at the third week is an independent risk factor for BPD in premature infants.Its combination with gestational age,PS administration,and duration of caffeine therapy provides certain predictive value for the occurrence of BPD.关键词
