The Impact of Rural Medical Service Accessibility on Elderly Self-Rated Health from the Perspective of Supply and Demand

Abstract
Rural medical service accessibility is closely related to health poverty alleviation and the consolida-tion of poverty alleviation achievements.Using panel data from the China Family Panel Survey from 2016 to 2020,this study empirically examines the impact of medical service accessibility on rural elderly self-rated health from both supply and demand perspectives.The findings reveal:First,supply-side accessibility has a significant positive effect on self-rated health.Hospitalization and frequent medical visits improve self-rated health while also reflecting disease severity,and are associated with reduced self-care ability and increased depressive symptoms;Second,de-mand-side accessibility significantly affects self-rated health.Hospitalization cost ratios,comprehensive cost indi-ces,and pension-related variables show a significant negative effect on self-rated health,and the regarding pensions may be related to differences in age structure and health baseline;Third,the supply-demand interaction term shows a significant negative effect,indicating that supply-demand mismatch weakens the health-promoting effects of both factors.It is recommended to optimize supply-demand matching mechanisms,improve primary medical service qual-ity,enhance medical insurance and pension systems,and reduce medical cost burdens for rural elderly.These efforts will improve their health status and consolidate health poverty alleviation achievements.关键词
