Application Features and Contemporary Value of the Purgative Method in"Rumen Shiqin"

Abstract
Zhang Congzheng,a physician affiliated with the pathogen-attacking school,demonstrated proficiency in treating diseases by eliminating pathogenic factors via the sweating method,emetic therapy,and purgative method.Among these,the purga-tive method was employed most frequently.In clinical practice,Zhang recognized"internal obstruction by excess pathogens,leading to inhibited free flow"as the core pathogenesis and adopted"unblocking the obstruction"as the therapeutic principle,thus applying the purgative method in a flexible manner.Rumen Shiqin documents multiple forms of the purgative method that possess distinct thera-peutic functions.These forms include purging fire with cold and cool medicinals,expelling water through purgation,eliminating accu-mulations,expelling toxins,and using foods for laxation.In terms of clinical application,Zhang's theory of the purgative method posits that physicians should clearly distinguish between cold and heat conditions and use medicinals with prudence.They should assess disease conditions accurately and apply purgation in an appropriate and moderate manner.Physicians should also conduct a detailed examination of the patient's constitution and customize treatment according to the individual.It is essential to ensure the complete elimination of pathogenic factors and exercise caution when using tonification after purgation.Moreover,when appropriate,physicians should substitute foods for medicinals and then provide dietary nourishment for recovery.In addition,Zhang comprehen-sively summarized the contraindications of the purgative method,highlighting that it should not be applied to cases of exterior patho-gens,pure deficiency syndromes,or critical illnesses.Zhang's concept of attacking and expelling obstructive excess pathogens exhibits certain similarities with modern purgative therapies,and thus holds research significance for the treatment of contemporary chronic and intractable diseases.His emphasis on prioritizing the elimination of pathogenic factors and opposing indiscriminate tonification also offers valuable guidance for rational medication use and the adjustment of contemporary health concepts.关键词
