Abstract
Objective To investigate the clinical features,diagnostic strategies,and therapeutic principles of chordoma,thereby enhancing clinicians' recognition of this rare disease entity.Methods We report the case of a 58-year-old male patient admitted in 2023 with a preoperative radiological diagnosis of invasive pituitary macroadenoma.The patient underwent neuroendoscopic transnasal-transsphenoidal resection of the lesion.Chordoma was confirmed postoperatively by histopathological and immunohistochemical(IHC)findings,and the relevant literature was systematically reviewed.Results The sellar mass was completely resected via neuroendos copic surgery.Postoperative pathology and IHC staining(positive for Brachyury,etc)confirmed chordoma.The patient's symptoms of ptosis and facial paralysis were significantly improved.However,postoperative adjuvant radiotherapy and targeted therapy were declined due to personal reasons.Conclusion Sellar chordoma exhibits localizing value through its clinical manifestations and imaging characteristics.Definitive diagnosis relies on pathological features and IHC markers.Treatment should prioritize maximal safe surgical resection combined with postoperative proton/heavy-ion radiotherapy to reduce the risk of recurrence.关键词
