TONG Guangdong's Approach to Treating Tympanites Based on the Principle of"Balancing Through Regulation and Removing Stagnation and Accumulated Waste"

Abstract
Hepatic ascites falls within the category of"tympanites"in traditional Chinese medicine(TCM).It is a refractory condition that occurs when chronic liver disease progresses to the decompensated stage of cirrhosis.Although conventional Western medical treatment can alleviate symptoms,the condition is prone to recurrence and has a poor prognosis.TONG Guangdong,a renowned TCM practitioner in Guangdong Province,has devoted decades of clinical experience to the field of liver diseases.He has deeply integrated the principle of"balancing through regulation and removing stagnation and accumulated waste"with the pathogenesis of tympanites,thereby establishing a distinctive system of pattern differentiation and treatment.Taking"balancing through regulation"as the fundamental therapeutic tenet,he harmonizes the liver to restore its compliant function,regulates the ascending and descending activities of the liver,lung,spleen,and stomach,and weighs the balances between pathogenic factors and healthy Qi,deficiency and excess,as well as cold and heat.In this way,he achieves the simultaneous regulation of the liver and spleen and the holistic balance of Qi,blood,Yin and Yang.He also employs the water-regulating strategies of"opening the sweat pore and promote diuresis"and using classical formulas,applies treatments such as dispersing the lung to expel retained fluid,promoting blood circulation to facilitate diuresis,warming yang to transform qi,and unblocking the bowels and promoting urination.Furthermore,he emphasizes the combined use of acupuncture and herbal medicine,as well as internal and external therapies.Throughout the treatment,he consistently adheres to the principles of supporting healthy Qi while eliminating pathogenic factors and addressing both the root and branch manifestations.He precisely controls the balance of tonification and elimination according to the stage of disease progression,providing valuable insights and references for the TCM clinical management of cirrhotic ascites.关键词
