Value of high-resolution MRI in evaluating the vulnerability of carotid artery plaques in young stroke patients

Abstract
Objective To explore the value of high-resolution MRI(HR-MRI)in evaluating the vulnerability of carotid artery plaques in young stroke patients.Methods A total of 83 young stroke patients admitted to Huai'an Hospital Affiliated to Xuzhou Medical University from February 2022 to December 2024 were enrolled as research subjects.According to the results of postoperative pathological examination,they were divided into vulnerable group(vulnerable plaques,n=45)and stable group(stable plaques,n=38).All patients underwent HR-MRI of carotid artery plaques.The differences in intraplaque components,HR-MRI related quantitative indicators and blood flow grades between the two groups were compared to analyze the application value of HR-MRI in evaluating carotid vulnerable plaques in young stroke patients.Results The levels of low-density lipoprotein cholesterol and fibrinogen in the vulnerable group were significantly higher than those in the stable group,while the level of high-density lipoprotein cholesterol was significantly lower(all P<0.05).The lumen area,fibrous cap thickness and the proportion of blood flow grade 0 in the vulnerable group were significantly lower than those in the stable group,while the necrotic core index,lipid core size,lipid core area,lumen stenosis rate and the proportion of blood flow grade 1 and 2 were significantly higher(all P<0.05).The area under the curve of HR-MRI for evaluating carotid vulnerable plaques was0.877,with a diagnostic accuracy of 91.11%,a sensitivity of 84.21%and a specificity of 87.95%.The diagnostic results of this examination were highly consistent with the pathological gold standard(Kappa=0.756,P<0.05).Conclusion HR-MRI can accurately analyze the internal components of carotid artery plaques in young stroke patients and has high value in evaluating carotid vulnerable plaques,which is worthy of clinical promotion and application.关键词
