Abstract
Objective To explore the correlation between plasma homocysteine(Hcy)and adiponectin(ADPN)levels and the degree of cognitive dysfunction in patients with hypertension.Methods A total of 180 patients with hypertension admitted to our hospital from June 2024 to June 2025 were selected and divided into the cognitive impairment group and cognitive normal group according to the presence of cognitive impairment.General data including gender,age,blood pressure,past disease history,smoking history and drinking history,as well as plasma indicators including Hcy,ADPN,total cholesterol,triglyceride,high-density lipoprotein cholesterol,low-density lipoprotein cholesterol,uric acid,fasting plasma glucose,2 h postprandial blood glucose,glycated hemoglobin and insulin(FINS)were collected and compared between the two groups.Pearson correlation analysis was used to explore the correlation between plasma Hcy,ADPN levels and the degree of cognitive impairment in patients with hypertension.Results The Montreal Cognitive Assessment score of 180 patients with hypertension was(24.35±2.96)points,among which 95 cases(52.78%)scored<26 points;according to the degree of cognitive impairment,there were32 cases of mild impairment(22~25 points),40 cases of moderate impairment(16~21 points)and 23 cases of severe impairment(≤15 points).Patients were divided into the cognitive impairment group(n=95)and cognitive normal group(n=85).There were statistically significant differences in age,years of education,FINS,Hcy and ADPN levels between the two groups(all P<0.05).Multivariate Logistic regression analysis showed that increased age,shortened years of education,elevated Hcy level and decreased ADPN level were independent risk factors for cognitive impairment in patients with hypertension(all P<0.05).With the aggravation of cognitive impairment,patients'Hcy level showed a gradient upward trend and ADPN level showed a gradient downward trend(all P<0.05).Pearson correlation analysis showed that Hcy level was negatively correlated with the degree of cognitive impairment in patients with hypertension(r=-0.762,P<0.05),and ADPN level was positively correlated with the degree of cognitive impairment in patients with hypertension(r=0.721,P<0.05).Conclusions Plasma Hcy and ADPN are independent risk factors for cognitive impairment in patients with hypertension.Clinically,attention should be paid to the changes of these two indicators to identify high-risk patients with cognitive impairment early and take targeted intervention measures.关键词
