锁骨远端骨溶解MRI征象的研究
Study on MRI signs of distal clavicular osteolysis

Abstract
Objective To investigate the MRI findings and key signs of distal clavicular osteolysis(DCO).Methods A consecutive series of 16 patients diagnosed with DCO clinically and radiologically,who underwent shoulder MRI at Beijing Jishuitan Hospital between January 2020 and June 2024,were selected as the case group.Another 16 patients with negative MRI findings were selected as the control group.Two expe-rienced physicians reviewed the images,recording the frequency and percentage of key imaging signs;impor-tant acromial morphological parameters,including the acromioclavicular joint space,acromial tilt angle,lateral acromial angle,and acromial index,were measured.Results The most frequent MRI findings in the DCO case group were bone marrow edema in the acromial end of the clavicle(16 cases,100.00%),soft tissue edema and/or joint fluid in the acromioclavicular joint(15 cases,93.75%),and bone defect of the acromial end of the clavicle(14 cases,87.50%).Notably,15 cases(93.75%)exhibited the"acromial sparing"sign.The acromio-clavicular joint space was significantly wider in the case group(P=0.002),and the lateral acromial angle was significantly larger(P=0.007).Conclusion The characteristic features of DCO are bone marrow edema and bone defect in the acromial end of the clavicle,along with soft tissue swelling in the acromioclavicular joint and/or joint space effusion,demonstrating the"acromial sparing"characteristic.DCO patients show a widened acromioclavicular joint space and an increased lateral acromial angle,and the alteration in the lateral acromial angle suggests an association between the occurrence of DCO and acromial morphology.关键词
