多学科团队模式对结直肠癌伴转移患者远期预后的影响
Impact of the multidisciplinary team model on the long-term prognosis of patients with colorectal cancer with metastasis

Abstract
Objective To explore the impact of the multidisciplinary team(MDT)model on the prog-nosis of patients with colorectal cancer with synchronous metastases.Methods A retrospective analysis was conducted on the clinical data of 755 patients diagnosed with colorectal cancer with metastasis in the hospital from January 2015 to December 2021.According to the implementation time of MDT,patients diagnosed from January 2015 to October 2016 were classified as the conventional treatment group(n=241),and those diag-nosed from January 2018 to December 2021 were classified as the MDT group(n=514).The treatment regi-mens and quality of life scores of the two groups were compared.Survival outcomes was analyzed using Kap-lan-Meier method,and the influencing factors of prognosis were evaluated using the Cox regression model.Results Compared with the conventional treatment group,the MDT group had higher rates of surgical treat-ment and neoadjuvant therapy,lower rates of chemotherapy/radiotherapy and palliative treatment,higher scores for role function,emotional function and social function,lower pain scores,and a longer median overall survival(OS)time(2.65 years vs.1.24 years),with statistically significant differences(P<0.05).The re-sults of multivariate Cox regression analysis showed that the participation in MDT(HR=0.468,95%CI:0.386-0.568)and gender(HR=0.776,95%CI:0.637-0.946)were independent influencing factors for the prognosis of patients(P<0.05).Conclusion The MDT model significantly prolonged the OS time for pa-tients with colorectal cancer and synchronous metastasis.关键词
