经皮肝胆道穿刺硬质胆道镜取石术治疗肝胆管结石的疗效分析
Efficacy analysis of percutaneous transhepatic cholangioscopy with rigid choledochoscope for calculus removal in the treatment of hepatolithiasis

Abstract
Objective:To compare the clinical efficacy of percutaneous transhepatic cholangioscopy(PTCS)versus laparosco-pic hepatectomy(LH)in the treatment of hepatolithiasis.Methods:A prospective cohort study was conducted.Clinical data of 60 patients with hepatolithiasis admitted between Aug.2021 and Aug.2023 were collected.Based on preoperative comprehensive assessments and patients'treatment preferences,30 patients underwent percutaneous transhepatic puncture rigid choledochoscope lithotomy and 30 patients underwent LH.The two groups were compared in terms of the levels of white blood cell,C-reactive protein,and transaminases before and after surgery,operative time,intraoperative blood loss,stone residual rate,time to first postoperative anal exhaust,incidence of postoperative complications,hospital stay and hospitalization cost.Results:The PTCS group had less intraoperative blood loss,shorter operative time hospital stay and shorter time to first postoperative anal exhaust,as well as lower hospitalization cost,incidence of post-operative complications,transaminase levels and inflammatory markers(white blood cell and C-reactive protein)than the LH group.Conclusions:Percutaneous transhepatic puncture rigid choledochoscope lithotomy shows superior advantages in reducing postoperative complications,improving transaminase and inflammatory markers(white blood cell and C-reactive protein),decreasing hospitalization costs and shortening hospital stay in the treatment of hepatolithiasis,which is conducive to postoperative rehabilitation of patients.关键词
