摘要

Abstract

Carbapenem-resistant Acinetobacter baumannii bloodstream infection(CRAB-BSI)represents a pressing clinical challenge due to its high mortality rate and limited therapeutic options.Through systematic review of existing guidelines and evidence-based medical data, this analysis indicates that successful treatment of carbapenem-resistant Acinetobacter baumannii bloodstream infection relies on early and effective combination anti-infective strategies, with pharmacokinetic/pharmacodynamic(PK/PD)-guided dose optimization representing the core approach to enhancing therapeutic efficacy.Current evidence supports that a therapeutic regimen based on high-dose,prolonged-infusion sulbactam combined with polymyxin B(with dosing adjusted according to the minimum inhibitory concentration)is the primary option with the most robust clinical data support. In regions where it is available, combination regimens containing cefiderocol also represent an important alternative.Although novel antimicrobial agents continue to emerge,high-quality studies specifically targeting carbapenem-resistant Acinetobacter baumannii bloodstream infection remain insufficient.Future therapeutic breakthroughs depend on conducting prospective studies focused on bloodstream infection, advancing MIC-and PK/PD-guided precision dosing, and integrating novel therapeutic modalities, ultimately leading to standardized treatment pathways.
