Effects of Tongdu acupuncture based on"warming yang and resolving depression"theory and Huafeng Dan on depression scores and levels of 5-hydroxytryptamine and homocysteine in patients with post-stroke depression

Abstract
Objective To observe the effects of Tongdu acupuncture based on the theory of"warming yang and resolving depression,"combined with Huafeng Dan on depression scores and serum levels of 5-hydroxytryptamine(5-HT)and homocysteine(Hcy)in patients with post-stroke depression(PSD).Methods A retrospective analysis was conducted on 72 patients with PSD treated in the Department of Acupuncture and Tuina,Affiliated Hospital of Traditional Chinese Medicine of Binzhou Medical University,from May 2021 to November 2023.According to different treatment regimens,the patients were divided into a treatment group(n=36)and a control group(n=36).Both groups received citalopram.The control group additionally received conventional acupuncture,while the treatment group received Tongdu acupuncture based on the theory of"warming yang and resolving depression"combined with Huafeng Dan.The Hamilton Anxiety Scale(HAMA),Hamilton Depression Scale(HAMD),Modified Barthel Index(MBI),National Institutes of Health Stroke Scale(NIHSS),and serum levels of Hcy and 5-HT before and after treatment were compared between the two groups.Adverse reactions were also recorded.Results After treatment,HAMA,HAMD,and NIHSS scores were significantly lower in both groups than before treatment(P<0.05),and all were lower in the treatment group than in the control group(P<0.05).After treatment,MBI scores increased in both groups,and the treatment group had a higher MBI score than the control group(P<0.05).After treatment,serum 5-HT levels increased in both groups compared with pre-treatment levels(P<0.05),and the treatment group had higher 5-HT levels than the control group(P<0.05).Serum Hcy levels decreased in both groups after treatment compared with before treatment(P<0.05),and the treatment group had lower serum Hcy levels than the control group(P<0.05).The incidence of adverse reactions was 13.89%(5/36)in the treatment group and 5.56%(2/36)in the control group;the difference between the two groups was not statistically significant(P>0.05).Conclusion Tongdu acupuncture based on the theory of"warming yang and resolving depression"combined with Huafeng Dan has significant efficacy in the treatment of PSD,helps alleviate depressive symptoms,regulates 5-HT and Hcy levels,and demonstrates a high level of safety.关键词
