疏肝化痰方治疗肝郁夹痰型持续性姿势感知性头晕伴焦虑抑郁状态的临床研究
Clinical Study of Shugan Huatan Formula in Treatment of Persistent Postural-Perceptual Dizziness with Anxiety and Depression of Liver Stagnation with Phlegm Retention Syndrome

Abstract
Objective:To explore the clinical efficacy of Shugan Huatan Formula in the treatment of patients with persistent postural-perceptual dizziness(PPPD)complicated with anxiety and depression state of liver stagnation with phlegm retention syndrome,and to investigate its underlying mechanism.Methods:A total of 85 patients diagnosed with PPPD of liver stagnation with phlegm retention syndrome complicated with anxiety and depression state were enrolled from the outpatient department of Encephalopathy Department,Shijiazhuang Hospital of Traditional Chinese Medicine,from December 2023 to December 2024.All patients were randomly divided into the control group(43 cases)and the treatment group(42 cases)using the random number table method.Patients in the control group were treated with Escitalopram Oxalate Tablets,while the treatment group was additionally given Shugan Huatan Formula granules on the basis of the control group,with a treatment course of 6 weeks.Before and after treatment,the TCM syndrome score,Dizziness Handicap Inventory(DHI)score,Niigata PPPD Questionnaire(NPQ)score,Self-rating Depression Scale(SDS)score,Self-rating Anxiety Scale(SAS)score,as well as serum levels of interleukin-6(IL-6)and brain-derived neurotrophic factor(BDNF)were compared between the two groups.The clinical efficacy was evaluated,and adverse reactions during treatment were compared.Results:After treatment,both groups presented significantly decreased TCM syndrome scores,DHI scores,NPQ scores,SDS scores,SAS scores and serum IL-6 levels,while the serum BDNF levels were significantly increased compared with those before treatment(all P<0.05).The treatment group showed superior improvement in the above indicators to the control group(P<0.05).The overall clinical efficacy of the treatment group was significantly better than that of the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:Shugan Huatan Formula can improve clinical symptoms,relieve anxiety and depression,and reduce the level of inflammatory factors in PPPD patients with liver stagnation and phlegm retention syndrome,which possesses high clinical safety.关键词
