Abstract

Objective To investigate the clinical efficacy of acupoint catgut embedding combined with medicinal cupping therapy in patients with diabetic peripheral neuropathy(DPN)and its effects on nerve repair and microcirculation.Methods A total of 60 patients with DPN admitted to the Department of Rehabilitation Medicine of the Seventh Affiliated Hospital of Xinjiang Medical University from February 2024 to February 2025 were enrolled and randomly divided into control group and observation group using a random number table method,with 30 patients in each group.Both groups received standard basic treatment.The control group received oral mecobalamin tablets(0.5 mg per dose,three times daily).The observation group received additional acupoint catgut embedding(once every 15 days)and medicinal cupping therapy(10 minutes per session,once every 3 days)on top of the conventional treatment.The treatment course for both groups was 6 months.After treatment,clinical efficacy was evaluated in both groups.The following parameters were observed before and after treatment:TCM pattern scores,Visual Analogue Scale(VAS)scores,Michigan Diabetic Neuropathy Score(MDNS),and levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(2hBG),and glycated hemoglobin(HbA1c).Motor nerve conduction velocity(MNCV)and sensory nerve conduction velocity(SNCV)of the median nerve and common peroneal nerve were measured using an electromyograph.Nailfold microcirculation status was assessed using a microcirculation diagnostic instrument.Serum levels of neurotrophic factors,including nerve growth factor(NGF),brain-derived neurotrophic factor(BDNF),and neurotrophin-3(NT-3),were checked by ELISA.Adverse reactions during the treatment period were recorded.Follow-up assessments of MDNS and VAS scores were conducted at 3 and 6 months post-treatment,and the recurrence rate was compared between the two groups.Results Compared with baseline values before treatment,both groups showed significant decreases in TCM pattern scores,VAS scores,MDNS scores,nailfold microcirculation scores,and levels of FBG,2hBG,and HbA1c after treatment(P<0.05),as well as significant increases in MNCV and SNCV of the median and common peroneal nerves,and in serum NGF,BDNF,and NT-3 levels(P<0.05).Compared with the control group,the observation group showed significantly greater reductions in TCM pattern scores,VAS scores,MDNS scores,nailfold microcirculation scores,and levels of FBG,2hBG,and HbA1c(P<0.05),and significantly greater increases in MNCV and SNCV of the median and common peroneal nerves,as well as in serum NGF,BDNF,and NT-3 levels(P<0.05).There was no statistically significant difference in the incidence of adverse reactions between the two groups(P>0.05).The total clinical effective rate in the observation group was significantly higher than that in the control group(P<0.05).Follow-up at 3 and 6 months post-treatment showed that MDNS and VAS scores in the observation group were significantly lower than those in the control group(P<0.05),and the recurrence rate was also lower in the observation group than in the control group(P<0.05).Conclusion Acupoint catgut embedding combined with medicinal cupping therapy can effectively relieve clinical symptoms and blood glucose levels,promote nerve function repair in DPN patients,and demonstrate good safety.Its mechanism of action may be related to upregulating the expression of serum neurotrophic factors and improving microcirculation status.
