基于国际妊娠合并糖尿病研究组织标准的妊娠期糖尿病危险因素meta分析
Meta-analysis of risk factors for gestational diabetes based on the Inter-national Association of Diabetes and Pregnancy Study Groups on gesta-tional diabetes mellitus

Abstract
Objective To explore the risk factors of gestational diabetes mellitus(GDM)and provide a basis for clinical pre-vention.Methods The system retrieved databases in both Chinese and English to collect studies on risk factors for GDM.The literature was screened according to standards,and evaluated for quality using the Newcastle-Ottawa scale(NOS)and the Agency for Healthcare Research and Quality(AHRQ)criteria.High-quality studies were included for meta-analysis.Results A total of 29 studies were included,involving 14 511 cases and 99 034 controls.The meta-analysis showed that 13 factors were significantly associated with GDM:age(OR=1.08,95%CI:1.07-1.09),systolic blood pressure(OR=1.02,95%CI:1.02-1.03),pre-pregnancy body mass index(OR=1.14,95%CI:1.13-1.16),increase in body mass index during pregnancy(OR=1.04,95%CI:1.02-1.06),family history of diabetes(OR=1.80,95%CI:1.62-1.99),history of GDM(OR=7.19,95%CI:6.11-8.46),glycated hemoglobin(OR=2.24,95%CI:1.54-3.27),insulin resistance index(OR=4.15,95%CI:2.31-7.47),total cholesterol(OR=2.62,95%CI:2.00-3.45),triglycerides(OR=1.71,95%CI:1.48-1.98),positive thyroid peroxidase antibody(OR=1.53,95%CI:1.17-2.00),25-hydroxyvitamin D deficiency(OR=1.57,95%CI:1.24-1.98),and smoking(OR=1.40,95%CI:1.16-1.69).Conclusion GDM is associated with a variety of physiological,metabolic and behavioral factors,and comprehensive screening is necessary to achieve early prevention.关键词
