针刺联合个性化影音疗法对脑卒中后情感障碍的临床效果
Clinical effect of acupuncture combined with personalized audio-visual therapy on post-stroke affective disorder

Abstract
Objective To explore the clinical effect of acupuncture combined with personalized audio-visual therapy on post-stroke affective disorder.Methods A total of 80 patients with post-stroke affective disorder admitted to Jingdezhen Hospital of Traditional Chinese Medicine from January 2023 to May 2025 were selected as the research objects.According to the random number table method,they were divided into the control group(n=40)and the observation group(n=40).The control group was treated with personalized audio-visual therapy,and the observation group was treated with acupuncture on the basis of the control group.The scores of traditional Chinese medicine symptoms,Hamilton depression scale(HAMD),Hamilton anxiety scale(HAMA),Pittsburgh sleep quality index(PSQI),modified Barthel index(MBI)and clinical global impression(CGI)(severity of illness[SI],global improvement[GI]and efficacy index[EI])were compared between the two groups.Event-related potential P300 parameters(latency and amplitude),brain-derived neurotrophic factor(BDNF)and neuron-specific enolase(NSE)levels were detected and compared between the two groups.The incidence of adverse reaction during treatment was counted and com-pared between the two groups.Results After intervention,the scores of traditional Chinese medicine symptoms(depression,anxiety,irritability,crying and laughing,sleep disorders)of the two groups were lower than those before intervention,and the scores of the observation group were lower than those of the control group,and the differences were statistically significant(P＜0.05).After intervention,the HAMA,HAMD and PSQI scores of the two groups were lower than those before interven-tion,and the MBI scores were higher than those before intervention,and the HAMA,HAMD and PSQI scores of the observation group were lower than those of the control group,and the MBI score was higher than that of the control group,and the differences were statistically significant(P＜0.05).After intervention,the SI,GI and EI scores of the two groups were lower than those before intervention,and the scores of the observation group were lower than those of the control group,and the differences were statistically significant(P＜0.05).After intervention,the latency of event-related potential P300 and serum NSE levels of the two groups were lower than those before the intervention,the amplitude of event-related potential P300 and serum BDNF levels were higher than those before the intervention,and the latency of event-related potential P300 and serum NSE levels of the observation group were lower than those of the control group,the amplitude of event-related potential P300 and serum BDNF level in the observation group were higher than those in the control group,and the differences were statistically significant(P＜0.05).There was no statistically significant difference in the total incidence of adverse reaction between the two groups during treatment(P＞0.05).Conclusion Acupuncture combined with personalized audio-video therapy can effectively improve the symptoms of depression and anxiety,sleep quality and activity of daily living in patients with post-stroke affective disorder,and its mechanism may be related to the regulation of event-related potential P300 parameters,BDNF and NSE levels.The treatment is safe and worthy of clinical application.关键词
