老年2型糖尿病患者血清胰高血糖素样肽-1对其并发糖尿病视网膜病变的辅助诊断价值
Auxiliary diagnostic value of serum GLP-1 for diabetic retinopathy in elderly patients with type 2 diabetes mellitus

Abstract
Objective To investigate the auxiliary diagnostic value of serum glucagon-like peptide-1(GLP-1)for diabetic retinopathy(DR)in elderly patients with type 2 diabetes mellitus(T2DM).Methods A total of 106 elderly patients with T2DM admitted to the hospital from January 2021 to December 2023 were selected as the study subjects and divided into a DR group(65 cases)and a non-DR group(41 cases)according to the presence or absence of DR.Additionally,50 healthy individuals who underwent physical examination at the physical examination center of the hospital during the same period were selected as the control group.Serum GLP-1 level was compared among the three groups.Multivariate Logistic regression analysis was performed to identify the risk factors for DR in elderly patients with T2DM.Receiver operating characteristic(ROC)curve was plotted to evaluate the auxiliary diagnostic efficacy of serum GLP-1 for DR in elderly patients with T2DM.Results Serum GLP-1 level in both the DR group and the non-DR group was found to be significantly higher than that in the control group(P<0.05),and serum GLP-1 level in the DR group was significantly higher than that in the non-DR group(P<0.05).Elevated systolic blood pressure(SBP),and elevated levels of fasting plasma glucose(FPG)and low-density lipoprotein cholesterol(LDL-C)were identified by multiva-riate Logistic regression analysis as risk factors for the development of DR in elderly patients with T2DM(P<0.05).When the optimal cut-off value of serum GLP-1 was set at 0.97 pmol/L,the area under the curve(AUC)of serum GLP-1 for the auxiliary diagnosis of DR in elderly patients with T2DM was 0.851(95%CI:0.808-0.944),with a sensitivity of 90.77%and a specificity of 78.05%.Conclusion Elevated SBP GLP-1,FPG and LDL-C are risk factors for the development of DR in elderly patients with T2DM.Serum GLP-1 has a certain auxiliary diagnostic value for DR in elderly patients with T2DM,which is worthy of clinical promo-tion.关键词
