The Value of Autologous Platelet-Rich Plasma Perfusion in Endometrial Regener-ation and Repair After Surgery for Moderate to Severe Intrauterine Adhesions

Abstract
Objective:To evaluate the clinical efficacy of autologous platelet-rich plasma(PRP)intrauterine perfusion in endometrial repair after transcervical resection of adhesions(TCRA)for moderate-to-severe intrauter-ine adhesions(IUA).Methods:From July 2023 to July 2024,seventy patients with moderate-to-severe IUA treated at Jiangxi Maternal and Child Health Hospital were randomly assigned to the PRP group(n=35)or the control group(n=35).Both groups underwent TCRA followed by hormonal therapy,while the PRP group received addi-tional intrauterine PRP perfusion immediately after surgery,and on postoperative days 7 and 14.Therapeutic effects were assessed viahysteroscopic scoring,menstrual improvement,endometrial thickness,clinical pregnancy outcomes and adverse reactions were recorded.Results:The PRP group exhibited significantly higher rates of u-terine cavity restoration(85.71%vs.60.00%,P=0.032),menstrual improvement(77.14%vs.48.57%,P=0.026)and thicker endometrium(7.97±1.19 mm vs.5.74±1.26 mm,P<0.001)compared to the control group.Al-though the clinical pregnancy rate did not reach statistical significance(57.14%vs.37.14%,P=0.082),the total live birth rate was significantly elevated in the PRP group(51.43%vs.28.57%,P=0.041).The incidence of ad-verse events was comparable between groups(28.57%vs.25.71%,P=0.788).Conclusions:Autologous PRP intrauterine perfusion effectively promotes the restoration of postoperative uterine anatomy and physiological func-tion in patients with moderate-to-severe IUA,significantly enhances menstrual improvement and live birth rates,and demonstrates a favorable safety profile.关键词
