艾灸对阳虚质乳腺癌化疗中性粒细胞减少的临床疗效及体质影响
Clinical efficacy of moxibustion in preventing and treating chemotherapy-induced neutrope-nia in breast cancer patients with yang-deficiency constitution and its impact on patient-reported outcomes and TCM constitution

Abstract
Objective To observe the clinical efficacy of moxibustion for chemotherapy-induced neutropenia(CIN)in breast cancer patients with yang-deficiency constitution after adjuvant chemotherapy,and its effects on patient-reported outcomes(PRO)and TCM constitution transformation scores.Methods Eighty early-stage breast cancer patients with yang-deficiency constitution(identified by TCM constitution assessment)undergoing anthracycline plus taxane chemotherapy were randomly divided into a moxibustion group(n=40)and a control group(n=40).The control group did not receive moxibustion.The moxibustion group received mild moxibustion once daily during each chemotherapy cycle at Shenque(CV 8),Zusanli(ST 36),and Sanyinjiao(SP 6)acupoints for 24 weeks(8 cycles).The control group received standard care.The following parameters were observed before and after treatment:1.CIN efficacy:absolute neutrophil count(ANC),neutropenia grade,time to neutropenia onset,and dosage of recombinant human granulocyte colony-stimulating factor(rhG-CSF).2.PRO:EORTC QLQ-C30 and QLQ-BR23 scores,adverse events(AEs),and safety indicators.3.TCM constitution transformation scores.4.Survival follow-up.Results During cycles 5-8,the moxibustion group showed higher periph-eral ANC than the control group(P<0.05).In cycles 4-6,the distribution of neutropenia grades was more favorable in the moxibustion group(P<0.05).In cycles 5,7,and 8,the timing of neutropenia onset was better in the moxibustion group.In cycle 4,rhG-CSF dosage was lower in the moxibustion group(P<0.05).Regarding PRO,moxibustion improved QLQ-C30 scores in physical,emotional,respiratory,and gastrointestinal domains(P<0.05).For QLQ-BR23,moxibustion ameliorated body image,arm symptoms,breast symptoms,and treatment side effects(P<0.05).Concerning AEs,moxibustion alleviated fatigue,weakness,hot flashes,nausea/vom-iting,diarrhea,constipation,and muscle pain(P<0.05).No significant differences were observed in liver/kidney function,blood lipids,or cardiac indicators(P>0.05).For TCM constitution,post-treatment yang-deficiency symptom scores and constitution transfor-mation scores were better in the moxibustion group(P<0.05).After a median follow-up of 45.1 months,no significant differences were found in disease-free survival(DFS)or overall survival(OS)between groups(P>0.05).Conclusion Moxibustion can increase periph-eral ANC,improve CIN,enhance PRO,reduce chemotherapy-related AEs,and ameliorate yang-deficiency constitution scores in breast cancer patients receiving anthracycline-taxane adjuvant chemotherapy.The moxibustion for neutrophil elevation approach is suitable for clinical promotion.关键词
