血必净注射液联合常规治疗对重症肺炎患者炎症风暴-凝血功能紊乱的影响
Retrospective study on the effect of Xuebijing Injection(血必净注射液)on inflammatory storm and coagulation dysfunction in patients with severe pneumonia

Abstract
Objective To investigate the effects of Xuebijing Injection(血必净注射液,XBJI)on inflammatory response and coagula-tion function in patients with severe pneumonia(SP).Methods A retrospective cohort study was conducted using medical records of SP patients treated at the Affiliated Yixing Hospital of Jiangsu University.All patients received conventional Western medicine treatment.They were divided into a non-exposure group(184 cases)and an exposure group(117 cases)based on whether XBJI was administered.IBM SPSS 26.0 was used for descriptive analysis of the screened patients'baseline data.Propensity score matching was performed based on baseline characteristics,resulting in 92 matched pairs(184 cases in total).Levels of C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6),white blood cell(WBC)count,D-dimer,prothrombin time(PT),activated partial thromboplastin time(APTT),fibrinogen(FIB),and platelet(PLT)count were compared between the two groups before treatment,and on days 7 and 14 of treatment.Multivariate logistic regression analysis was performed with pre-and post-treatment FIB levels as covariates to identify factors influencing coagulation function.Results After 14 days of treatment,both groups showed significant improvement in inflammatory and coagulation indicators compared with baseline.The total effective rate was 98.9%in the exposure group.In the non-exposure group,CRP,PCT,IL-6,PT,FIB,and PLT levels after 14 days of conventional treatment showed statistically significant improvement com-pared to baseline(P<0.01).In the exposure group,CRP,PCT,IL-6,WBC,FIB,and PLT levels after 14 days of XBJI treatment showed statistically significant improvement compared to baseline(P<0.05,P<0.01).Compared with the non-exposure group at the same time point,the exposure group showed statistically significant improvements in CRP,PCT,IL-6,WBC,PT,FIB,and PLT levels after 14 days of treatment(P<0.05,P<0.01).Multivariate logistic regression indicated that CRP and APTT levels were the main fac-tors influencing coagulation function in SP patients.Conclusion The combination of XBJI with conventional Western medicine treatment can further improve inflammatory response and coagulation dysfunction in SP patients,with clinical efficacy superior to that of conven-tional treatment alone.关键词
