A therapeutic framework for essential hypertension:integrating"three-type,eight-state"pattern differentiation with"state-target concurrent regulation"based on the"vascular disten-sion"theory

Abstract
Essential hypertension(EH)is characterized by challenges within traditional Chinese medicine(TCM),such as inconsis-tent nomenclature and unclear pathogenesis,which impede advances in its clinical diagnosis and treatment.Based on the"Vascular dis-tension"theory from the Yellow Emperor's Canon of Medicine(黄帝内经),this concept is established as the standardized TCM disease name,locating the condition in the"vessels"and defining its core pathogenesis as"disharmony of qi and blood."Building on this foun-dation,this study integrated traditional qi-blood theory,modern constitution identification,and metabolomics,constructing a"Three-type"pattern differentiation framework for"Vascular distension"and validating its biological basis through metabolomics technology.Furthermore,by incorporating Academician TONG Xiaolin's scholarly thought of"State-Target-Cause-Effect,"this study further expanded the framework into a"Three-type,Eight-state"pattern differentiation and treatment system and introduced a"State-Target Concurrent Regulation"treatment strategy.This strategy employs a core formula for"Regulating Congestion,Draining Water,Protect-ing Vessels"as the foundation,complemented by classical formulas for holistic state regulation and supplemented with targeted medici-nals for precise treatment.Preliminary clinical validation has demonstrated promising therapeutic efficacy.This constructed integrated TCM system of"Pathogenesis-Constitution-Metabolism"may provide a novel theoretical basis and practical approach for the TCM diag-nosis and treatment of hypertension.关键词
