Implementation Status and Optimization Suggestions for Home-Based Pharmaceutical Care of Chronic Diseases in 16 Districts of Beijing

Abstract
Objective To systematically investigate the implementation status of home-based pharmaceutical care,pri-marily in the form of medication therapy management(MTM)in Beijing,providing references for optimizing service models,im-proving chronic disease management mechanisms,and establishing talent development systems.Methods Structured inter-views and questionnaire surveys were conducted to collect feedback from medical institutions and patients on the implementation of home-based pharmaceutical care services for chronic diseases models,with a focus on identifying key issues.Evidence-based optimization strategies were proposed using empirical data.Results A total of 152 interview reports and 2 539 valid question-naires were collected(97.65％validity rate).The results revealed regional imbalances in chronic disease home-based pharmaceu-tical services in Beijing,with significant disparities in service availability between urban and rural areas,uneven allocation of pharmaceutical human resources,and high demand for pharmaceutical services among middle-aged and elderly chronic disease patients,coupled with low awareness levels.Conclusions Optimizing patient engagement,pharmacist training,and policy formulation constitutes critical pathways for advancing the in-depth development of pharmacist-led chronic disease pharmaceutical care.关键词
