超声评估子宫内膜容积及血流参数对薄型子宫内膜患者内膜增厚治疗反应性的预测价值
Ultrasound assessment of endometrial volume and blood flow parameters in predicting treatment response in patients with thin endometrium

Abstract
Objective:To investigate the predictive value of pre-treatment three-dimensional(3D)ultrasound-measured en-dometrial volume(EV)and blood flow parameters for the responsiveness of thin endometrium(TE)patients to endometrial thick-ening treatment.Methods:A prospective cohort study design was adopted,enrolling a total of 95 patients diagnosed with TE(endometrial thickness<7 mm during hormone replacement or ovulation induction cycles)in our hospital's gynecology depart-ment between January 2022 and June 2024,who were scheduled to undergo endometrial thickening treatment.Prior to treat-ment,all patients underwent transvaginal 3D power Doppler ultrasound to measure endometrial thickness(EMT),endometrial vol-ume(EV),vascularization index(Ⅵ),flow index(FI),vascularization flow index(VFI),as well as bilateral uterine artery pulsatility index(UtA-PI)and uterine artery resistance index(UtA-RI).Based on whether post-treatment EMT reached ≥7 mm,patients were divided into a good-response group(n=63)and a poor-response group(n=32).Differences in baseline clinical data and ul-trasound parameters between the two groups were compared.Binary Logistic regression analysis was used to screen for inde-pendent predictors,and the predictive performance of each parameter was evaluated using receiver operating characteristic(ROC)curves.Results:Univariate analysis showed that EV,VI,and VFI were significantly higher in the good response group than in the poor response group(P<0.05),while UtA-PI and UtA-RI were significantly lower(P<0.05).Multivariate Logistic re-gression analysis indicated that EV and UtA-PI were independent predictors of treatment responsiveness.A predictive model combining EV and UtA-PI achieved an area under the curve(AUC)of 0.866,which was superior to using EV alone(AUC=0.832)or UtA-PI alone(AUC=0.777).Conclusion:Pre-treatment EV and UtA-PI are effective independent indicators for pre-dicting the responsiveness of TE patients to endometrial thickening treatment.The combined application of these ultrasound parameters can assist in formulating individualized clinical treatment plans.关键词
