彩色多普勒超声联合胎盘体积检测对胎儿生长受限的诊断价值
The diagnostic value of color Doppler ultrasound combined with placental volume measurement for fetal growth restriction

Abstract
Objective:Exploring the diagnostic value of color Doppler ultrasound combined with placental volume(PV)detection for fetal growth restriction(FGR).Methods:A retrospective selection of 105 singleton pregnant women admitted to our hospital from March 2023 to February 2025 was conducted as the study subjects.Based on the occurrence of FGR,they were divided into the FGR group(n=46)and the non-FGR group(n=59).The clinical data of the two groups of pregnant women were compared,and the ultrasound parameters and PV data of the pregnant women were collected.Multivariate Logistic regression analysis was used to analyze the influencing factors of fetal growth restriction,and ROC curve analysis was performed to evaluate the diagnostic value of ultrasound parameters and PV for fetal FGR.Results:There were no statistically significant differences in maternal age,BMI,gestational age,gravidity,and parity between the two groups(P>0.05),but the neonatal birth weight in the FGR group was significantly lower than that in the non-FGR group(P<0.05).The fetal umbilical artery(UA)blood flow parameters,including the systolic-to-diastolic velocity ratio(S/D),resistance index(RI),and pulsatility index(PI),were significantly higher in the FGR group;moreover,the PV in the FGR group was significantly smaller than that in the non-FGR group(P<0.05).The fetal middle cerebral artery(MCA)blood flow parameters RI and PI were significantly lower in the FGR group compared to the non-FGR group(P<0.05);however,there was no statistically significant difference in the peak systolic velocity(PSV)of fetal blood flow parameters between the two groups(P>0.05).The AUC values for S/D(UA),RI(UA),PI(UA),RI(MCA),PI(MCA),and PV in diagnosing fetal FGR were 0.768,0.801,0.729,0.763,0.819,and 0.741,respectively,with a combined diagnostic AUC of 0.924.Conclusion:The blood flow parameters of UA and MCA,as well as PV,all exhibit high diagnostic efficacy for the occurrence of FGR in fetuses,with the highest diagnostic efficacy observed when these three factors are combined.关键词
