Pancreatic end-to-end anastomosis following central pancreatectomy
摘要
Abstract
Central pancreatectomy adheres to the concept of organ-preserving surgery,but has drawn significant attention due to its high incidence of postoperative pancreatic fistula.For distal reconstruction after central pancreatectomy,pancreaticojejunostomy is commonly performed,while pancreaticogastrostomy is used in some cases.However,both procedures are technically complex and carry an increased risk of pancreatic fistula;moreover,because they alter the physiological outflow tract of pancreatic juice,any resulting fistula is inevitably a mixed fistula.In contrast,pancreatic end-to-end anastomosis not only restores the anatomical conti-nuity and physiological function of the digestive tract,but also involves a relatively simpler surgical procedure.When properly executed,it can significantly reduce both the incidence and severity of postoperative pancreatic fistula,thereby making it the ideal reconstruction method following central pancreatectomy.Consequently,precise mastery of the specific technical steps of end-to-end anastomosis and appropriate placement of a pancre-atic duct stent are key determinants in preventing pancreatic fistula and serve as critical guarantees for ensuring optimal surgical quality in central pancreatectomy.关键词
