Abstract
Objective To investigate the detection rate of body dysmorphic disorder in public hospital plastic surgery patients,analyze its correlation with anxiety and depression,compare body dysmorphic disorder differences across gender,education and dissatisfied body parts,and identify independent related factors for body dysmorphic disorder severity.Methods Selected from the Affiliated Hospital of Nanjing University of Chinese Medicine between April 2025 and January 2026,1,070 patients scheduled for aesthetic plastic surgery were enrolled as study subjects.Demographic data including gender and education level were collected.The Body Dysmorphic Disorder-Yale-Brown Obsessive Compulsive Scale,Self-Rating Anxiety Scale,and Self-Rating Depression Scale were used for questionnaire survey.Univariate analysis was used to compare body dysmorphic disorder score differences among patients with different characteristics,multiple linear regression was used to analyze related factors of body dysmorphic disorder,and mediating effect test was used to examine the mediating role of depression between dissatisfied body parts and body dysmorphic disorder.Results Among 1 070 patients,the total score of Body Dysmorphic Disorder-Yale-Brown Obsessive Compulsive Scale was 10.82±7.55,the detection rate of body dysmorphic disorder was 22.43%,the detection rate of anxiety was 14.30%,and the detection rate of depression was 22.34%.Body dysmorphic disorder was significantly positively correlated with Self-Rating Anxiety Scale and Self-Rating Depression Scale(r=0.255,0.322).Univariate analysis showed that patients with associate degree or below education and those dissatisfied with nose or maxillofacial contour had higher body dysmorphic disorder scores(P<0.05).Regression analysis showed that Self-Rating Depression Scale was an independent related factor for body dysmorphic disorder(β=0.29,R2=0.83).Mediating effect analysis showed that the mediating effect of depression between dissatisfied body parts and body dysmorphic disorder was not significant(95%CI included 0).Conclusion The detection rate of body dysmorphic disorder in public hospital plastic surgery patients is 22.43%.Depression is highly correlated with body dysmorphic disorder(R2=0.83).Depression and body dysmorphic disorder are parallel comorbidities rather than a mediating mechanism.Lower education level and dissatisfaction with nose or maxillofacial contour are associated with higher body dysmorphic disorder scores.It is recommended to conduct routine psychological screening before surgery,and multidisciplinary collaborative management with the psychology department should be provided for high-risk patients.关键词
