Effect of ursodeoxycholic acid on SARS-CoV-2 infection in patients with autoimmune liver disease

Abstract
[Objective]To investigate the effects of ursodeoxycholic acid(UDCA)on clinical symptoms and laboratory parameters of patients with autoimmune liver disease(AILD)combined with SARS-CoV-2 infection.[Methods]Patients with AILD followed up from December 20,2022 to January 20,2023 at Xiangya hospital were selected as the research subjects,SARS-CoV-2 infection and UDCA medication data in patients with AILD were collected through questionnaires,and the changes in liver function and complement levels before and after SARS-CoV-2 infection were tracked through the hospital's electronic medical record system,and SPSS 25.0 was used for statistical analysis.[Results]The UDCA group showed a significantly lower rate of SARS-CoV-2 infection than that in the non-medication group(P<0.05).For patients with SARS-CoV-2 infection,UDCA did not significantly improve infection symptoms or liver injury symptoms(P>0.05).After infection,liver enzymes[alanine aminotransferase(ALT),aspartate aminotransferase(AST),alkaline phosphatase(ALP),glutamyl transpeptidase(GGT),α-L-fucosidase(AFU)]and complement levels(C3,C4)in AILD patients were significantly reduced(P<0.05).Compared with the non-UDCA treatment group,the UDCA treatment group showed a significant decrease in cholestasis-related indicators[total bilirubin(TBil),direct bilirubin(DBil),bile acid(BA)](P<0.05).[Conclusion]SARS-CoV-2 infection does not exacerbate liver injury in AILD patients but may be associated with complement system(CS)activation and comsumption.UDCA can reduce the risk of SARS-CoV-2 infection,improve cholestasis and mitigate further liver function damage,suggesting its potential clinical protective role.关键词
