Drug Utilization Evaluation of Budesonide,Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol Based on Analytic Hierarchy Process-Weighted Technique for Order Preference by Similarity to Ideal Solution Method

Abstract
Objective To promote the rational use of Budesonide,Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol in the treatment of chronic obstructive pulmonary disease(COPD).Methods A total of 116 medical records of COPD inpatients treated with Budesonide,Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol from April to September 2025 were extracted from the hospital information system.Relevant guidelines,consensuses,literature,and package inserts were used as references,combined with common irrational drug use situations in the hospital.A rational drug use evaluation panel was established,and two rounds of Delphi expert consultation were conducted.Finally,a drug utilization evaluation(DUE)standard for Budesonide,Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol in COPD inpatients was established.The analytic hierarchy process(AHP)was used to analyze the weight of evaluation indicators in the standard,and the weighted technique for order preference by similarity to ideal solution(TOPSIS)method was used to construct a rationality evaluation matrix for each medical record.Data standardization was performed to obtain the optimal solution(positive ideal solution)and the worst solution(negative ideal solution),and the relative closeness(Ci)of each medical record to the positive ideal solution was calculated to evaluate drug use rationality.Results An evaluation system consisting of seven first-level indicators and nine second-level indicators was established.Among them,the weights of indication,device operation and patient education were relatively higher,which were 0.325 9,0.210 5 and 0.136 3,respectively.The average Ci value of 116 medical records was 0.564 3,among them,there were 6.03%of patients with rational drug use(0.8≤Ci<1),59.48%with basically rational drug use(0.6≤Ci<0.8),and 34.48%with irrational drug use(Ci<0.6).In the evaluation of second-level indicators,contraindications,dosage,device operation,and combination therapy showed no irrational prescriptions.Irrational drug use was mainly concentrated in indications,patient education,and adverse reaction monitoring.Conclusion The DUE standard for Budesonide,Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol established using the AHP-weighted TOPSIS method can be used for clinical drug use rationality evaluation.The problem of irrational use of Budesonide,Glycopyrronium Bromide and Formoterol Fumarate Inhalation Aerosol in COPD inpatients in this hospital is prominent,and it is necessary to strengthen drug use supervision and intervention in terms of indications,patient education,and adverse reaction monitoring to promote rational drug use.关键词
