Abstract
Objective To explore the application effect of early rehabilitation nursing based on the patient-centered eCASH(early Comfort using Analgesia,minimal Sedatives,and maximal Humane care)concept in patients with intensive care unit-acquired weakness(ICU-AW).Methods A total of 82 patients with ICU-AW admitted at the First Affiliated Hospital of Xiamen University from November 2022 to November 2024 were enrolled in the study.They were assigned into an observation group and a control group using a random number table method,with 41 cases in each group.The control group received routine nursing,while the observation group received routine nursing combined with early rehabilitation nursing based on the eCASH concept.The nutritional indicators(hemoglobin[Hb],prealbumin[PA],and albumin[ALB]),motor function,muscle strength,and activities of daily living were compared between the two groups.Results After 2 and 4 weeks of nursing,the Medical Research Council(MRC)scores,the Barthel index,and the Fugl-Meyer Assessment scores in both groups were significantly higher than those before nursing,and the scores in the observation group were significantly higher than those in the control group(P<0.05).The improvement of nutritional indicators(Hb,PA,and ALB)in the observation group was significantly better than that in the control group(P<0.05).After 4 weeks of nursing,the observation group had significantly shorter length of total hospital stay,mechanical ventilation duration,and ICU length of stay compared with the control group(P<0.05).Conclusion The implementation of early rehabilitation nursing based on the eCASH concept can effectively enhance muscle strength,improve motor function and nutritional status,and shorten the duration of mechanical ventilation and length of hospital stay in patients with ICU-AW.It is worthy of clinical promotion and application.关键词
