基于循证理念的老年髋部骨折患者术后谵妄防控培训方案的构建与应用
Construction and application of an evidence-based training program for the prevention and control of postoperative delirium in elderly patients with hip fractures

Abstract
Objective To construct an evidence-based training program for the prevention and control of postoperative delirium in elderly patients with hip fractures and to evaluate its application effects from the perspectives of nurses and patients.Methods A non-simultaneous historical controlled design was adopted,the implementation time is segmented with July 2024 as the boundary.A total of 40 nurses from the orthopedic ward of the Affiliated Suzhou Hospital of Nanjing Medical University were selected as training participants using continuous sampling.Clinical data of 102 elderly patients undergoing hip fracture surgery from January to December 2024 were collected.Patients admitted from January to June 2024(n=51)were set as the control group,during which the nurses received routine postoperative delirium prevention and control training.Patients admitted from July to December 2024(n=51)were set as the observation group,during which the evidence-based training program for postoperative delirium prevention and control was constructed and implemented.The nurses'knowledge of postoperative delirium was compared before and after the evidence-based training.The incidence and duration of delirium,length of hospital stay,and patient satisfaction with nursing care were compared between the two groups.Results After the evidence-based training,the scores of all dimensions of the Chinese version of the Delirium Knowledge Test for Intensive Care Nurses were significantly higher than those before training(P<0.05).The incidence of delirium in the observation group(5.88％)was lower than that in the control group(19.61％),the duration of delirium and length of hospital stay in the observation group were shorter than those in the control group,and the patient satisfaction rate with nursing care in the observation group(90.20％)was higher than that in the control group(72.55％),with statistically significant differences(P<0.05).Conclusion The evidence-based training program for postoperative delirium prevention and control in elderly patients with hip fractures can improve nurses'ability to prevent and control delirium,reduce the incidence of postoperative delirium,shorten the duration of delirium and length of hospital stay,and increase patients'nursing satisfaction rate.关键词
