Abstract
Acne is a common chronic inflammatory skin disease in clinical practice,known as"lung-wind pimple"in traditional Chinese medicine(TCM).The disease is prone to occur in adolescents,also prone to recurrence,and its disfigurement often causes significant psychological and social burdens for patients.Modern medicine considers its pathogenesis complex,with bacterial infection as the key link.Although routine symptomatic treatment such as antibiotics has a certain effect,it is difficult to eradicate the disease.As a treasure our traditional culture,TCM has unique characteristics in the understanding and treatment of acne.As early as in Huangdi Nei jing(《黄帝内经》),the pathogenesis related to"Yu Nai Cuo"(郁乃痤)was proposed.After the continuous supplementation by later medical experts,modern TCM often identifies acne as three main syndromes:lung channel heat,spleen and stomach damp-heat,and phlegm-dampness stasis.The treatment is often based on clearing heat and cooling blood,detoxifying and resolving masses,promoting blood circulation and removing blood stasis,and invigorating the spleen and resolving dampness.However,clinical observations have found that the efficacy of routine therapy is not satisfactory for nodular and cystic acne.As a TCM characteristic external treatment method,the fire needling therapy has unique effects such as warming and dredging the merdians,opening the portal to dispel pathogenic factors,and using heat to induce heat.Based on years of clinical practice,Chief Physician ZHANG Ling has a deep grasp of the TCM theories of"stagnation-induced acne"and"treating heat with heat",and applies the fire needling therapy to the treatment of acne,especially nodular and cystic acne for which routine therapy is ineffective.She holds that intractable intermingling state of phlegm,blood stasis,heat and toxin exists in the deep layer of such skin lesions.Simple therapy of clearing heat and cooling blood barely penetrates to the lesion site,and it is necessary to resort to the effects of fire needling in warming the meridians,unblocking the collaterals,dissipating stagnation and expelling pathogenic factors to directly reach the focus,break up the stagnation and eliminate toxin,so as to achieve a curative effect.This paper aims to systematically summarize Chief Physician ZHANG Ling's clinical experience in treating acne with fire needling under the guidance of the"Treating Heat with Heat"theory,and a clinical case is attached herein for verification.关键词
