Fertility preservation and assisted reproductive technologies in young patients with breast cancer:current status and future perspectives

Abstract
Fertility preservation in women of reproductive age with breast cancer has become a critical issue in the field of oncofertility.With advances in cancer diagnosis and treatment,the demand for fertility preservation among young patients has increased substantially.Correspondingly,available strategies have evolved from embryo cryopreservation alone to a diversified technological framework that includes oocyte cryopreservation and ovarian tissue cryopreservation.Key clin-ical studies,particularly the POSITIVE trial,have provided high-level evidence supporting the short-term safety of in-terrupting endocrine therapy to achieve pregnancy in patients with hormone receptor-positive breast cancer,thereby influ-encing clinical practice.Current controversies and research hotspots focus on the optimal timing of pregnancy,individual-ized selection of fertility preservation strategies,and reproductive risk management in patients with hereditary breast canc-er,such as BRCA mutation carriers.A multidisciplinary approach is essential for the successful implementation of fertility preservation.However,several challenges remain,including insufficient standardization of clinical practice,limited ac-cessibility to advanced technologies,and a lack of long-term follow-up data.Future efforts should emphasize the inte-gration of precision medicine,optimization of longitudinal patient management,and improvement of supportive policy frameworks,in order to achieve a balanced outcome between effective cancer treatment and fertility goals.关键词
