年轻乳腺癌患者生育力保存及辅助生殖38例
Fertility preservation and assisted reproductive outcomes in young women with breast cancer:a retrospective study

Abstract
Objective To study the effectiveness of fertility preservation and assisted reproduction in young breast cancer patients.Methods Clinical data from 38 young breast cancer patients undergoing fertility preservation(oocyte cry opreservation or embryo cry opreservation)and assisted reproduction at the Reproductive Medicine Center of Sun Yat-sen Memorial Hospital between 2010 and December 2024 were retrospectively analyzed,involving a total of 53 cycles.Pa-tients undergoing IVF/ICSI treatment due to tubal or male factors served as the control group.Ovarian stimulation out-comes and assisted reproductive outcomes were compared between the two groups.Results There were no significant differences between the breast cancer group and the control group in the number of retrieved oocytes,mature oocytes,available embryos,high-quality embryos,blastocyst formation number,or blastocyst formation rate.However,the high-quality embryo rate was significantly lower in the breast cancer group than in the control group(23.36%vs.39.09%,P=0.010).Comparable ovarian stimulation outcomes were observed between the luteal-phase long protocol and antago-nist protocol.In addition,no statistically significant differences were found in estradiol levels on trigger day,number of re-trieved oocytes,or mature oocyte count between subgroups receiving letrozole supplementation at doses of 2.5 mg/day and 5 mg/day during ovarian stimulation.Seven patients completed breast cancer treatment and subsequently underwent assis-ted reproductive treatment.Among five fresh embryo transfer cycles,three resulted in full-term live births.Among six frozen-thawed embryo transfer cycles,including one patient carrying a BRCA2 mutation who underwent preimplantation genetic testing,three resulted in full-term live births and one resulted in early miscarriage.Conclusion Assisted repro-ductive technology provides effective options for fertility preservation and infertility treatment in young patients with breast cancer.关键词
