血清MMP11和TGF-β1在前列腺癌病理分期及预后评估中的应用
Clinical application of serum MMP11 and TGF-β1 in pathological staging and prognosis evaluation of prostate cancer

Abstract
Objective To evaluate the expression patterns of serum matrix metalloproteinase 11(MMP11)and transforming growth factor β1(TGF-β1)in prostate cancer(PCa)patients and their associations with pathological stag-ing and prognosis,and to investigate the prognostic predictive value of their combined assessment.Methods A total of 116 patients with histologically confirmed PCa were enrolled.Serum levels of MMP11 and TGF-β1 were measured at di-agnosis.Comparisons were made between PCa patients and benign prostatic hyperplasia(BPH)controls.Patients were stratified into high-and low-expression groups based on the median serum levels of MMP11 and TGF-β1.Associa-tions with clinicopathological features were analyzed.Kaplan-Meier survival curves and Cox proportional hazards models were applied to identify independent prognostic factors.Receiver operating characteristic(ROC)curves were plotted to as-sess the predictive performance of MMP11,TGF-β1,and their combination.Results Serum MMP11 and TGF-β1 levels were significantly elevated in PCa patients compared with BPH controls(P<0.05),and increased progressively with advancing TNM stage(P<0.05).High MMP11 expression was associated with unfavorable clinicopathological fea-tures,including higher PSA and CRP levels,advanced clinical stage,increased lymph node metastasis,and larger tumor diameter(all P<0.05).High TGF-β1 expression showed a similar adverse profile.Kaplan-Meier analysis demonstra-ted significantly lower overall survival in patients with high MMP11 or TGF-β1 expression.Multivariate Cox analysis con-firmed that high MMP11(HR=3.723)and high TGF-β1(HR=2.692)were independent predictors of poor prognosis after adjusting for TNM stage,lymph node metastasis,and PSA.ROC analysis showed that the area under the curve(AUC)for MMP11,TGF-β1,and their combination in predicting prognosis were 0.715,0.704,and 0.783,respec-tively.Conclusion Serum MMP11 and TGF-β1 levels are significantly elevated in PCa patients and increase with dis-ease stage.Their high expression is associated with adverse clinicopathological characteristics and poor survival,and both serve as independent prognostic indicators.Combined assessment of MMP11 and TGF-β1 enhances prognostic discrimi-nation in PCa,offering potential clinical utility.关键词
