Heterogeneity trajectory of uncertainty in illness and anxiety and depression in parents of premature infants:Latent categories and influencing factors

Abstract
Objective:To explore the potential categories of developmental trajectories of uncertainty in illness,anx-iety and depression in parents of premature infants,and to analyze their influencing factors.Methods:The parents of premature infants admitted to the neonatal department of our hospital from December 2023 to July 2025 were select-ed as the research objects by convenient sampling method.The uncertainty in illness was assessed within 72 hours af-ter admission(T1),1 week after admission(T2),1 day before discharge(T3),and 1 month after discharge(T4).The latent variable growth mixture model was used to identify and name the potential categories of uncertainty in illness.Univariate and multivariate logistic regression analysis were further used to explore the independent influencing fac-tors of differences between categories,and to analyze the relationship between anxiety and depression and uncertainty in illness.Results:A total of 200 questionnaires were distributed and 194 valid questionnaires(97.00％)were recov-ered.In this study,three potential categories of disease uncertainty trajectories of 194 parents of premature infants were identified by the growth mixture model:Persistent high uncertainty type(17％),gradual remission-adaptation type(32％)and persistent low uncertainty type(51％).Logistic regression analysis showed that gestational age at birth(＜32w),education level(high school and below),anxiety and depression scores were independent influencing factors of potential categories of uncertainty in illness among parents of different preterm infants(P＜0.05).Conclu-sion:There are certain individual differences in the development trajectory of uncertainty in illness of parents of pre-mature infants,which are divided into persistent high uncertainty type,gradual remission-adaptation type and per-sistent low uncertainty type,which are affected by gestational age,education level,anxiety and depression.关键词
