宣白承气汤加减治疗重症肺炎痰热壅肺证的临床疗效及其对患者炎症反应的影响
Clinical Efficacy of Modified Xuanbai Chengqi Decoction in the Treatment of Severe Pneumonia with Syndrome of Phlegm-Heat Obstructing Lung and Its Impact on Inflammatory Reaction of Patients

Abstract
Objective To investigate the clinical efficacy of modified Xuanbai Chengqi decoction in the treatment of severe pneumonia with syndrome of phlegm-heat obstructing lung and its impact on inflammatory reaction of patients.Methods A total of 98 severe pneumonia patients with syndrome of phlegm-heat obstructing lung treated in Beijing Chuiyangliu Hospital from August 2023 to August 2025 were selected as the study subjects.Using the random number table method,the patients were divided into the routine treatment group(49 cases)and the Xuanbai Chengqi decoction group(49 cases).Patients in the routine treatment group received routine treatment,while patients in the Xuanbai Chengqi decoction group received modified Xuanbai Chengqi decoction in addition to the routine treatment group.Both groups were treated for 7 days.The clinical efficacy,TCM syndrome scores,serum inflammatory factors(IL-6,TNF-α,and CRP),T lymphocyte subsets(CD4+and CD8+T lymphocyte cell fraction,CD4+/CD8+T lymphocyte cell ratio)and blood gas analysis indicators(SaO2,PaO2,and PaCO2)before treatment and after 7 days of treatment were compared between the routine treatment group and the Xuanbai Chengqi decoction group,and adverse reactions of the two groups were observed.Results The clinical efficacy of the Xuanbai Chengqi decoction group was better than that of the routine treatment group(P<0.05).After treatment,the scores of fever,cough,yellow sticky sputum with blood,shortness of breath and rough breathing,chest tightness and irritability,and dry mouth and thirst in the Xuanbai Chengqi decoction group were lower than those in the routine treatment group(P<0.05).After treatment,the serum levels of IL-6,TNF-α and CRP,and CD8+T lymphocyte cell fraction in the Xuanbai Chengqi decoction group were lower than those in the routine treatment group,while the CD4+T lymphocyte cell fraction and CD4+/CD8+T lymphocyte cell ratio were higher than those in the routine treatment group(P<0.05).After treatment,SaO2 and PaO2 in the Xuanbai Chengqi decoction group were higher than those in the routine treatment group,and PaCO2 was lower than that in the routine treatment group(P<0.05).There was no statistically significant difference in the total incidence of adverse reactions between the routine treatment group and the Xuanbai Chengqi decoction group(P>0.05).Conclusion Modified Xuanbai Chengqi Decoction has a certain clinical efficacy in the treatment of severe pneumonia with syndrome of phlegm-heat obstructing lung,and it can effectively reduce the TCM symptoms and inflammatory response,improve the immune function and blood gas analysis indicators,and with high safety.关键词
