清肺化痰平喘方治疗支气管扩张症急性加重期肺热壅盛证临床疗效观察
Clinical efficacy of decoction for clearing lung,resolving phlegm to calm panting on acute exacerbation of bronchiectasis with syndrome of heat accumulated in lung

Abstract
Objective It is to observe the clinical efficacy of decoction for clearing lung,resolving phlegm to calm pan-ting in the treatment of patients with acute exacerbation of bronchiectasis with syndrome of heat accumulated in lung.Meth-ods A total of 64 patients with acute exacerbation of bronchiectasis with syndrome of heat accumulated in lung treated in the Respiratory Department of Shunyi Hospital of Beijing Traditional Chinese Medicine Hospital from December 2023 to Decem-ber 2024 were randomly divided into an observation group and a control group,with 32 patients in each group.The control group was treated with conventional western medicine,while the observation group was treated with decoction for clearing lung,resolving phlegm to calm panting on the basis of therapy in the control group,both groups were treated for 2 weeks.The scores of traditional Chinese medical syndrome,scores of cough and sputum assessment questionnaire(CASA-Q),ar-terial blood gas analysis(PaO2),24-hour sputum volume,inflammatory indexes[white blood cell count(WBC),neutro-phil ratio(NEUT%),C-reactive protein(CRP),procalcitonin(PCT),interleukin-6(IL-6)]of patients in the two groups were compared before and after treatment,the clinical efficacies were evaluated,and the occurrence of adverse reac-tions was recorded in both groups.Results During the treatment process,4 cases were lost in the control group and 0 cases were lost in the observation group.Compared with before treatment,the scores of traditional Chinese medical syndrome,24-hour sputum volume,WBC,NEUT%,as well as serum levels of CRP,PCT and IL-6 of patients in both groups were significantly reduced after treatment(all P<0.05),while the scores of CASA-Q in cough symptoms,cough impact,spu-tum symptoms,sputum impact,and PaO2 were significantly increased(all P<0.05).The scores of traditional Chinese medical syndrome and 24-hour sputum volume were significantly lower,while the scores of CASA-Q in the above items and PaO2 were significantly higher in the observation group than those in the control group after treatment(all P<0.05),there was no statistically significant difference in inflammatory indicators between the two groups(all P>0.05).The total effec-tive rate of clinical treatment in the observation group was significantly higher than that in the control group[93.75%(30/32)vs 71.43%(20/28),P<0.05].There was no statistically significant difference in the incidence of adverse reactions between the control group and observation group[14.29%(4/28)vs 6.67%(2/32),P>0.05].Conclusion Decoction for clearing lung,resolving phlegm to calm panting combined with conventional treatment of western medicine can signifi-cantly alleviate the clinical symptoms such as cough,phlegm and wheezing,improve the hypersecretion of airway mucus,increase the body's oxygen carrying capacity,alleviate inflammatory reaction in patients with acute exacerbation of bronchi-ectasis with syndrome of heat accumulated in lung,and its clinical efficacy is better than that of western medicine alone with good safety.关键词
