侧卧位直接上方入路在股骨头缺血性坏死患者全髋关节置换术中的应用
Application of the Direct Superior Approach in the Lateral Position in Total Hip Arthroplasty for Patients with Avascular Necrosis of the Femoral Head

Abstract
Objective To investigate the clinical application effect of the direct superior approach in the lateral position in total hip arthroplasty for patients with avascular necrosis of the femoral head.Methods A retrospective analysis was conducted on 132 patients with avascular necrosis of the femoral head who underwent total hip arthroplasty in the 903 Hospital between March 2020 and May 2025.According to different surgical approaches,the patients were divided into an observation group(n=68,treated with the direct superior approach in the lateral position)and a control group(n=64,treated with the traditional posterolateral approach).Perioperative indicators,levels of inflammatory stress factors[C-reactive protein(CRP),erythrocyte sedimentation rate,interleukin-6(IL-6)],Harris Hip Score(HHS),and the overall incidence of complications were compared between the two groups.Results The observation group had less intraoperative blood loss than the control group(P<0.05),and shorter operative time,incision length,hospital stay,and postoperative time to ambulation than the control group(P<0.05).At one week after surgery,the levels of CRP,erythrocyte sedimentation rate,and IL-6 in both groups were lower than those before surgery(P<0.05),and the levels in the observation group were lower than those in the control group(P<0.05).At three months after surgery,the HHS scores in all dimensions were higher in both groups than before surgery(P<0.05),but no statistically significant difference was found between the two groups in any dimension(P>0.05).The overall incidence of complications in the observation group and the control group was 4.41％and 15.63％,respectively,with the observation group showing a significantly lower rate(P<0.05).Conclusion The application of the direct superior approach in the lateral position in total hip arthroplasty for patients with avascular necrosis of the femoral head can effectively optimize perioperative indicators,alleviate surgical stress response,and reduce the risk of complications,while achieving hip function recovery comparable to that of the traditional posterolateral approach.It is a safe and efficient surgical approach.关键词
